Ibogaine drug interactions

Ibogaine has a first step intense liver metabolism. "Ibogaine is O-demethylated to noribogaine primarily by cytochrome P4502D6 (CYP2D6). An enzyme kinetic examination of ibogaine O-demethylase activity in pooled human liver microsomes suggested that two (or more) enzymes are involved in this reaction"(Mash et al).
 This group of enzimes is used extensively by a lot o regular medications and some suplements or food. The problem with this medications is that they prolongue the QT and produce cardiac arrithmias the same as Ibogaine does. 
The risk for cardiac complications increase when one or more of this drugs are used: CYP2D6 blockers: 
Antiarrithmic drugs: metoprolol; propafenone; timolol 
Antidepressant: amitriptilyne, clomipramine,desipramine, imipramine, paroxetine,venlafaxine 
Antipsicotic: haloperidol, risperidone, thioridazine, aripiprazole
Others: codeine,dextromethorphan,duloxetine, flecainide, mexiletine, ondansetron, tamoxifen, tramadol.
Drugs that prolongue QT: Amiodarone, Azithromycin Zithromax®; Bepridil Vascor®; Chloroquine Aralen®; Chlorpromazine Thorazine®;Citalopram Celexa®; Clarithromycin Biaxin®;Disopyramide Norpace®; Dofetilide Tikosyn®.Droperidol; Erythromycin;Escitalopram Lexapro; Flecainide; Tambocor® Halofantrine Halfan®; Haloperidol Haldol® Methadone Moxifloxacin Avelox® Pentamidine NebuPent® Pimozide Orap® Procainamide Pronestyl®; Quinidine Quinaglute® Sevoflurane Ulane® 

Grape Fruit Juice  and other supplements can produce the prolonged qt interval, there are groups of drugs that affect ibogaine and include antifungal medications, HIV treatment drugs, Omeprazol and other gastric drugs and some antihistamines.

It is very important you tell your doctor all the medications and supplements you are taking.
